Do radiant heating products have polarity?

Electric-resistance heating cables and systems have no polarity. This includes all current WarmlyYours heating elements.

If the defogger is smaller than the mirror, how far into the excess of mirror will this defog?

The defogging is concentrated in the area of the heating element, but may spread out as the mirror warms up over time. There are too many variable atmospheric conditions that make it impossible to quote a fixed distance of heat spread away from the element. That is why the defogger must be placed in the area of the mirror that requires defogging.

What kind of adhesive do I use to attach the mirror defogger to the mirror?

None - it is peel and stick (comes with an adhesive backing).

Can I sandwich the defogger between the mirror wood backing and the mirror?

As long as the wood is not glued or attached with any form of mastic to the defogger, yes.

How long does the mirror defogger take to get to 104°F / 40°C?

Note that it may never get all the way to that temperature because different mirrors and environments will affect the final result, thereby affecting the length of time it takes to achieve temperature.
